Approach the Church of the Plague and enter through a broken wall on Elden Ring Runes the eastern side. Inside, you'll find:

    Millicent, who is in dire need of the Unalloyed Gold Needle. Give it to her to save her life and officially start her questline.

    A Sacred Tear, an invaluable item used to upgrade your Flask of Crimson Tears.

By interacting with Millicent, you'll unlock one of the most compelling questlines in the game, which eventually leads to powerful rewards like unique spells and talismans.
